Certificate Fees

If you need to obtain a Birth Certificate for an individual born in Lucas County or a Death Certificate for an individual who passed away in Lucas County, you may obtain them from the Toledo-Lucas County Health Department.

The fee for certified copies of birth or death certificates is $25 per copy. Methods of Payment include cash, check or money order.  Due to changes in State law, we can no longer provide uncertified copies of birth or death certificates.

How to Order

You may order copies of certificate by mail, in person or on line with a credit card.

To order by mail, print a Birth Certificate or Death Certificate  form.  Mail the completed forms along with payment for each certificate and a self-addressed stamped envelope to Vital Statistics at the Toledo-Lucas County Health Department.  Upon receipt of your request, your certificates will be mailed back to you within 2 business days. Please allow 3 additional business days for the return mail.

To otain a certificate in person, come to the Vital Statistics Office at the Toledo-Lucas County Health Department.  In most cases your certificate will be issued while you wait.

For your convenience, you can process online requests through VitalChek Network, Inc. - an independant company that the Toledo-Lucas County Health Department has arranged with to provide you this service.  Vital Chek  may be reached through their web site.  An additional fee is charged by VitalChek for this service.  All major credit cards are accepted, including American Express, Discover, MasterCard or Visa.
