1. How do I find the Shots 4 Tots clinic schedule?

2. How do I register for WIC?

To register for WIC, simply dial (419) 213-4422 and you will be given instructions on how to register and be directed to the appropriate location. Also, you may access our WIC Eligibility flyer.

3. How do I get my medical / shot (immunization) record?

4. How do I get a skin test for tuberculosis?

Skin testing is offered every week day except for Thursdays, from 8:30 am – 11:30 am and 1:00 pm – 3:30 pm.

On the first Friday of each month, the clinic will be from 10:00 am– 11:30 am.

There is a $20 fee for each skin test.
